	Proof of Canadian Citizenship (Photocopy of Certificate of birth in Canada or Certicate of Canadian Citizenship)
	Background information on applicant (description of your disability, education, employment, etc.)
	Outline of research project (subject, significance, description of how research will be undertaken)
	Letter of support for this application

For your application to be considered, all of the above information must be sent to the Office of the Clerk of the Subcommittee on the Status of Persons with Disabilities and must be postmarked on or before 29 April 2005.
