With regard to the review conducted by Transport Canada following the fatal recreational vehicle accident in July 2024 near West Perth, Ontario, involving a child and the operation of a slide-out ("bump-out") mechanism, and subsequent actions taken by Transport Canada: (a) on what date did Transport Canada initiate and complete its review into the safety of recreational vehicle slide-out mechanisms; (b) what data, incident reports, or stakeholder consultations were considered as part of this review, including any input from manufacturers, safety experts, or the families affected; (c) what were the key findings and conclusions of the review, specifically regarding the safety risks posed by slide-out mechanisms; (d) why did Transport Canada determine that no regulatory changes or mandatory safety standards were required following the review; (e) why did Transport Canada decide to rely on voluntary compliance by recreational vehicle manufacturers rather than implementing mandatory safety requirements; (f) what steps is Transport Canada taking to inform recreational vehicle owners and the public about the potential hazards associated with slide-out mechanisms; (g) which manufacturers have been notified of the potential hazards, and what response, including notification of the implementation of voluntary safety improvements, did Transport Canada receive in response from each manufacturer; (h) has Transport Canada conducted or commissioned any risk assessments quantifying the likelihood of similar incidents occurring in the future, and, if so, what were the findings; (i) is Transport Canada planning to issue a recall, safety bulletin, or mandatory retrofit requirement for existing recreational vehicles equipped with similar slide-out mechanisms, and, if so, what is being planned; (j) if the answer to (i) is no, why not; and (k) did Transport Canada engage with or consult any United States authorities, including the National Highway Traffic Safety Administration, regarding the safety of recreational vehicle slide-out (bump-out) mechanisms and, if so, (i) on what dates did such engagement occur, and what information was exchanged, (ii) what differences were identified between Canadian and United-States regulatory approaches, standards, or enforcement practices, (iii) how did those differences inform Transport Canada's decision not to implement mandatory regulatory changes?
